Duplicate Diploma

There will be a small statement at the bottom of the diploma stating this is a replacement diploma. This does not show when the document is framed.
Your diploma will be ordered from information provided on this form; please review for accuracy. In order to keep cost low, your order will be processed once the University has reached the minimum number of required transactions for printing purposes. Therefore, orders outside of graduation timeframes may be delayed. Once the order has been placed by the University, it may take 3-4 additional weeks to receive your diploma.
