Duplicate Diploma

Alumni may order a replacement duplicate diploma below if their diploma is lost or damaged.

The cost for a duplicate diploma is $30
You should allow a minimum of 4 weeks' lead time.
Diplomas can be mailed to alumni or picked up.
Administrative signatures may differ from those which originally appeared when the diploma was first issued.

ATTENTION! This form is for alumni who have already graduated. Graduating seniors who have not yet received their diploma should use the Declaration of Intent to Graduate form.
