The Natatorium only membership includes access to the Natatorium during open hours and Water Aerobic classes.
If you are a new member-
Once your membership is processed you will receive a membership card. This card is required for signing into the facility. You may pick up your card at the Wellness Center front desk. Should you lose or damage your card there will be a $5 charge for a replacement card.
Thank you for choosing to use the facilities and services of the Department of Campus Recreation and Wellness. Our goal is to assist you in enhancing your health and well-being. Participation in activities sponsored by the Center carry with them some personal risk. We request your cooperation in maintaining both you and our safety by reading and signing the following.
I understand that every effort will be made by the Department of Campus Recreation and Wellness to provide quality services and safe facilities. However, I recognize that by participating in the Department's activities programs, and services, there exists some potential health risks. I may experience the possibility of physiological complications, including, but not limited to: muscular or joint injury, lightheadedness, fainting, abnormal blood pressure, chest discomfort, nausea, disorder of heart rhythm, and very rare instances of heart attack, stroke or death. I acknowledge my obligation to immediately inform a supervising employee of any unusual discomfort or symptom that I may suffer from my participation. I understand that I may stop any activity or procedure if I so desire, and that I may also be requested to stop and rest by a supervising employee who observes any symptoms of distress or abnormal response. I understand that part of the risk involved in undertaking any activity is relative to my own state of health and to the awareness, care and skill with which I conduct myself in activity. I acknowledge that I am under the general care of a physician and based on my physician’s evaluation, my health status is appropriate for exercise participation. I acknowledge that is my choice to participate in the activities, services and programs of the Department of Campus Recreation and Wellness and am willing to assume all risks which could occur by reason of my participation. I also acknowledge the rules posted and accept any consequences which may result from not following said rules including but not limited to being asked to leave, and or losing privileges.
I understand that the activities and services offered by the Department of Campus Recreation and Wellness are conducted by personnel whose skills and competencies will vary according to their training and experience and that no claim is made to offer assessment or treatment of a physical condition by those who are not employed to provide such professional services. I assume full responsibility for my choice to use or apply, at my own risk, any of the information or instruction I receive.
I voluntarily give consent to inclusion of data concerning my health and fitness status in a research data bank which may be used by the Department of Recreation and Wellness to investigate the relationship between various aspects of lifestyle and health. I understand that these data used for scientific research will receive only impersonal statistical treatment with my right of privacy protected. None of my data will be revealed in an individualized form to another person without my prior consent. Further, I recognize that I can discontinue participation at any time without any penalty of any kind.
I understand that I may ask questions or request further information about activities, services, or facilities within the Department of Campus Recreation and Wellness at any time. I have read, understood and agree to contents of this informed consent agreement in its entirety. All questions have been answered to my satisfaction.
*Memberships cannot be canceled or extended except for extenuating circumstances once purchased. Any cancelations or extensions must be approved by the Director and will be taken on a case by case basis.
