AGREEMENT & RELEASE OF LIABILITY WAIVER:
In consideration of being permitted to participate in any program hosted by Campbell University Campus Recreation and use any facility operated by Campbell University, Incorporated (“Campbell”), and in consideration of the voluntary nature of such participation and use, I hereby release, hold harmless, and forever discharge Campbell, its employees and agents, from any and all liability, claims, demands, actions, and causes of actions whatsoever arising out of or related to any loss, property damage, or personal injury, including death, that may be sustained by me or to any property belonging to me, while participating in such activity.
 
I acknowledge that I have read and agree that I am using these Facilities at my own risk and discretion. I acknowledge that participating in any physical activity and use of a pool facility presents inherent risk of injury or even death, that I accept as part of my voluntary participation and/or observation of these activities. I understand that if I do not abide by any Campus Recreation policy, or fail to adhere to Campus Recreation staff directions, then I may be prohibited from using Campus Recreation Facilities. I hereby agree to expressly assume and accept any and all risks of injury or death.
 
I do hereby further declare myself to be physically sound and suffering from no condition, impairment, disease, infirmity, or other illness that would prevent my participation except as hereinafter stated. I acknowledge that I have either been given permission by my physicians to participate, or that I have decided to participate and use these Facilities without physician approval and do hereby assume all responsibility for my participation and utilization of Campus Recreation activities and Facilities.
 
Campbell University assumes no responsibility for injuries received during Intramural activities. All participants are reminded that participation is voluntary. I affirm that I have both accident and medical insurance coverage in my name either through Campbell University, parents, guardians, or through a personal policy outside of Campbell University, which will cover any medical expenses that I may incur.
