Complete this application to apply for or renew a UW library borrowers card. Please review the terms and conditions prior to submission, including information on loan limits, financial responsibility and access restrictions.
After you submit the application, an email will be sent to you within 2 business days, informing you of the status of your application. Further proof or identification may be needed. If you do not receive an email, or have questions about the process, please contact Library Account Services.
Corporate/organizational cards are issued to a principal borrower who is responsible for the account. Specify “by bearer” to make the card transferable between all employees. Principal cards are $70 for 3 months, or $200 for a year. Additional proxy cards can be purchased for $70 each, with the same expiration date as the principal card.
