Request for Continuing Education Records

Request for duplicate certificates and other continuing education records. Only the individual who is certified may submit the request. Certificates are the sole property of the certified individual.

There is a fee of $10 per certificate.

Allow 10-14 days for the certificate to be electronically sent. Please consider your request carefully before submitting a form and payment, as there are no refunds for this service.
