The Duplicate Completion Document Request form authorizes College of Western Idaho (CWI) Workforce Development (WD) staff to search for the requested document and reissue a copy to the student. A completion document is any official certificate, card, or letter originally issued through WD.
To receive a Duplicate Completion Document:
By paying for this search you acknowledge that you understand that payment is not a guarantee of a completion document and the $10.00 per document fee is non-refundable. A completion document is defined as an official certificate, card or letter from CWI Workforce Development. Please note: Request processing may take up to two weeks from receipt of form submission and will be sent via US mail.
